Couchiching First Nation is seeking a dedicated and compassionate Community Health Representative to support the health and well-being of our community. This full-time, permanent role focuses on planning and delivering culturally appropriate health promotion, prevention, and education programs to improve community health outcomes.

Reporting to the Health Manager, the Community Health Representative will coordinate health initiatives, provide client support and referrals, maintain accurate health records, and collaborate with internal teams and external health partners. The ideal candidate will have a strong background in health promotion, experience working with First Nation communities, and a genuine commitment to supporting culturally safe health services.

Applicants must meet the following criteria:
  • Bachelor’s degree in Nursing, Health Sciences, or related field with 2 years of experience in health promotion; or a Diploma in a Health-related field with 6 years of work experience in health promotion.
  • Familiarity with Couchiching First Nation and its families.
  • Awareness of community health issues and programs related to First Nations.
  • Understanding of First Nation governance, culture, traditions and health beliefs is essential.
  • Ability to speak Ojibway or a willingness to learn the language is considered an asset
  • Excellent interpersonal, organizational, planning, and time management skills.
  • Also, the ability to develop health programs and prevention activities.
  • Commitment to maintain the highest level of confidentiality at all times.
  • Further, the ability to work collaboratively within the Health team and with health service providers.
  • Excellent interpersonal abilities with strong verbal and written communication skills.
  • Proficient with computers with the ability to learn new software.
  • First Aid/CPR Certification or willingness to be certified.
  • Additionally, the ability to provide a Vulnerable Sector Check.
  • Valid G level driver’s licence and access to a vehicle.
  • Finally, willingness to work flexible hours.

Salary commensurate with education and experience. All applicants will be subject to a Criminal Record Check (with Vulnerable Section Query), and Child Abuse Registry Check.
