The Alzheimer Society of Thunder Bay is looking for a resourceful, experienced, and passionate individual to join their team! In this vital role, the Dementia Connect Navigator serves as the first point of contact for all Alzheimer’s and/or Dementia related calls while reporting directly to the Executive Director. Moreover, the successful candidate will ensure timely support for immediate needs and facilitate virtual and in-person education and group support programs accessible to individuals across the region.

Pay: From CA$60,000.00 per year

As a Dementia Connect Navigator, you will play a crucial role in connecting clients with essential resources and compassionate care.

Key Responsibilities:
  • Firstly, receive all incoming calls for the Dementia Support Line and provide immediate support and information
  • Plan, implement, facilitate, and evaluate group education series annually to ensure accessible participation and barrier-free access for all
  • Additionally, promote relationships with related community specialists and organizations
  • Supporting clients and care partners in the development of coping strategies that support overall well-being
  • Accordingly, assist clients with navigating difficult emotions and situations related to Alzheimer’s and/or Dementia
  • Ensure that client consents, privacy, and confidentiality are maintained in compliance with legislation, professional standards/regulations and internal policies
  • Facilitate timely referrals to mental health supports and services available within the Society and/or community
  • Serving as a short-term emotional support for clients during wait list for other more comprehensive mental health supports and services (up to three months)
  • Collect, maintain, and report required quantitative and qualitative data to support monitoring, evaluation and reporting
  • Keep informed about Alzheimer’s disease and other dementias, clinical manifestations, behaviours, current care practices, treatment options, placement options, community resources, and relevant legislation
  • Assist with developing policies, procedures, and support resources
  • Perform other duties as required
Location:

Serving the District of Thunder Bay and all rural towns and communities, inclusive of First Nation communities as outlined in the organization’s service area.

Qualifications:
  • Degree in Social Work, graduate certificate in counselling, or another regulated health care designation
  • A master’s level education with current registration as a Canadian Certified Counsellor of the College of Psychologists of Ontario is considered an asset
  • Moreover, 3-5 years’ experience offering direct therapeutic communication and support within the health and/or social service sector
  • Experience in coaching, counselling, service coordination, or crisis response
  • Furthermore, experience supporting clients with complex mental health concerns
  • Similarly, experience building partnerships and collaborating with external organizations
  • Knowledge of therapeutic modalities that support clients’ ability to move through stress and distress and build resilience (CBT, coaching, etc.)
  • Advanced verbal and written communication skills
  • Fluency in French or another language considered an asset
  • Proficiency with Microsoft office, case management software, an care coordination systems
