Couchiching First Nation is seeking a permanent full-time Jordan’s Principle Navigator to join their Health team. The Jordan’s Principle Navigator reports directly to the Jordan’s Principle Coordinator and carries out the Jordan’s Principle Program for community-based service in the Couchiching First Nations Community as well as within in the town of Fort Frances and surrounding area. The Jordans Principle Navigator supports Couchiching First Nation by addressing existing gaps for service for children and their families through the Jordan’s Principle Child First Initiative.
Duties & Responsibilities:
- Help ensure families receive timely access to health and social systems.
- Guide children and families through a wide range of services in a client-driven and collaborative manner, ensuring access to multi-disciplinary resources as needed.
- Provide assessments within scope of practice and assist with referrals for assessments.
- Liaise and work collaboratively with community service providers, collateral agencies, organizations, and elders.
- Serve as a point of contact and deliver both educational sessions and informational support to agency staff regarding Jordan’s Principle.
- Work in collaboration with finance clerks to ensure accurate and timely processing of financial claims for Jordan’s Principle.
- Participate in community-based health committees/programs and in service unit meetings.
- Attend and participate in profession-centered meetings, seminars, and conferences.
- Further, maintain complete, accurate, legible and timely client files using the approved format.
- Ensure confidentiality and safe keeping of all documents and records.
- Complete weekly, monthly, quarterly and annual statistics, correspondence, and reports.
- Also, stay current with all relevant legislation, documentation, and professional literature related to Jordan’s Principle to support ongoing development and best practices.
- Perform other related duties as required.
Qualifications & Experience:
- Bachelor of Social Work Degree or Social Service field, or a related profession.
- Two years’ experience coordinating and/or managing programs and health related services.
- One year of direct experience working with children and families.
- Additionally, experience working with aboriginal people, organizations and communities.
- Ability to speak an Anishinaabe language is considered an asset.
- Strong knowledge of Anishinaabe culture, with an understanding of the issues affecting Anishinabek children and families.
- Understanding of First Nations health concerns, with the ability to apply this knowledge in developing as well as implementing programs that address community needs.
- Adaptability and the ability to foster a multidisciplinary, team-based approach to integrated service delivery.
- Ability to demonstrate initiative, optimism, discretion, tact, self-assurance, dependability problem solving, and leadership skills.
- Ability to provide coverage across all Health programs, where appropriate training has been provided and where required qualifications are met.
- Excellent interpersonal abilities with strong verbal and written communication skills.
- Commitment to maintain the highest level of confidentiality and professionalism at all times.
- Ability to provide a Vulnerable Sector Check.
- Valid G level driver’s licence and access to a vehicle.
- Willingness to travel and to work flexible hours.
