Summary:

Under the direction of the Director of Home & Community Care, the Home and Community Care Coordinator manages the day-to-day operations of the home care program for the Lac Seul First Nation community. In this vital role, the Home and Community Care Coordinator ensures culturally appropriate, client-centered services deliver support for the health, independence, and well-being of community members in their homes. Moreover, as the Home and Community Care Coordinator, you will lead program coordination while maintaining cultural safety and humility in all aspects of service delivery.

Location: Lac Seul, ON
Term: Full-time position
Salary: $37.19/hour *based on qualifications and experience.
Hours: 8:30 a.m. – 4:30 p.m., 35hrs/week

Home and Community Care Coordinator Key Accountabilities and Qualifications:
  • Firstly, a Diploma or degree in Nursing, Social Work, or a related health care discipline
  • In addition, a minimum three (3) years’ experience in a health care or home care setting; supervisory experience preferred
  • Experience working with Indigenous communities is an asset
  • Familiarity with Ontario’s home and community care standards and regulations
  • Understanding of Indigenous health and wellness frameworks, including cultural safety and humility
  • Knowledge of local community resources and networks
  • Strong interpersonal, communication, organizational, and time-management skills
  • Moreover, proficiency in Microsoft Office Suite, scheduling software, and data entry
  • Client-centered approach, cultural sensitivity, problem-solving, dependability, integrity, and adaptability
  • Valid driver’s license and access to reliable transportation
  • Willingness to participate in home visits, community engagement, and occasional evening/weekend work
  • Finally, a current Vulnerable Sector Check required
