The Alzheimer Society of Thunder Bay is looking for a motivated, experienced, and driven individual to join their team!
The First Link Care Navigator (FLCN) is responsible for the delivery of an inclusive, no barrier, responsive, first point of contact experience for all referrals received from across the Region and are recognized as first point of access or “go-to” person for all individuals experiencing changes to their cognitive and/or brain functioning.
The FLCN strives to ensure that every person has timely access to information, learning opportunities and support to achieve their optimal level of wellbeing and functioning when managing changes to their cognitive functioning or in someone they support. While the primary focus of services is directed towards the public and Care Partners the FLCN also support the needs of people actively living with a dementia related condition whether they have a formal diagnosis nor not.

Job Description
Job Title: First Link Care Navigator
Reports To: Executive Director
Status: Full-time position (35 hours per week)
Key Responsibilities:
- Facilitate consistent outreach to potential referrals sources building meaningful relationships that foster timely referral pathways.
- Offer Alzheimer and Dementia related information and support to all callers.
- Offer immediate one-time information, emotional and distress support to callers.
- Meeting the caller where they are and offering emotional support in the moment.
- Work with the caller to alleviate stressors and concerns and answer questions about Alzheimer and Dementia related symptoms.
- Support positive mental health of callers.
- Implement consistent opportunities for public education and group support.
- Serve as a direct intake pathway for the First Link Care Navigation Services.
- Facilitate care according to the FLCN service standards for registered clients.
- Offer education series and support groups for care partners as needed.
- Perform other duties as required.
Requirements:
- Must have use of personal vehicle in safe working condition.
- Required to travel extensively across the region as part of the role.
- Must carry a valid class G Driver’s license.
- Must carry $2 Million liability coverage.
- Must provide evidence of professional registration and carry professional liability coverage.

- Advanced communication skills especially in the realm of active and intentional listening
- Exceptional interpersonal skills, including shared decision-making and facilitation
- Ability to prioritize workload and manage competing tasks
- Ability to take initiative and be resourceful
- Excellent problem-solving and change management skills
- Proficiency in technology (e.g.: Microsoft office and case management and care coordination systems)
- Demonstrated ability to work independently and within a team
- Ability to forge strong partnerships across the care continuum
- Experience as a coach, counsellor, educator and service navigator is required
- Ability to facilitate education sessions and lead support groups
- Expertise and experience in cultural sensitivity and diversity
- Ability to speak French or other languages an asset
Salary:
This position pays $60,000 per annum, with vacation and additional holiday time off, flexibility, competitive benefits, RRSP match, and more!
Location:
This position is either home-based from Geraldton or Longlac, with travel required to service the surrounding communities, or based in Thunder Bay, with frequent travel expected to serve the following communities, including but not limited to: Nipigon, Schrieber, Terrace Bay, Marathon, Manitouwadge, Geraldton, Longlac, and the surrounding First Nation communities. Preference will be given to candidates residing in this area. Travel is expected to be around 65%, with mileage and commute time compensated.
