Job Summary:

Under the direction of the Director of Home & Community Care, the Nurse Social Navigator supports the navigation of healthcare services for Lac Seul First Nation members. Moreover, this role assists patients and families in accessing resources, coordinating care, and providing emotional and practical support throughout the healthcare journey. Furthermore, the Nurse Social Navigator promotes culturally grounded, client-centered care that aligns with community values, health priorities, and provincial standards.

Location: Lac Seul, ON
Term: Full-time position
Salary: $40.90/hour *based on qualifications and experience.
Hours: 8:30 a.m. – 4:30 p.m., 35hrs/week

Key Accountabilities and Qualifications:
  • Firstly, a Registered Nurse (RN) with a current and valid license
  • Equally important, a Bachelor’s degree in Nursing, Health Science, or related field preferred
  • Minimum of three (3) years clinical nursing experience, preferably in community or Indigenous health settings
  • Subsequently, experience in patient navigation, case management, or social support services is an asset
  • Demonstrated experience working with Indigenous communities or organizations
  • Broad understanding of Ontario’s healthcare system and community-based care frameworks
  • Knowledge of trauma-informed care, culturally safe practices, and holistic approaches to health
  • Awareness of social determinants of health affecting First Nations communities
  • Strong organizational, communication, and interpersonal skills
  • Experience with electronic medical record systems and health information management tools
  • Ability to prepare documentation, reports, and care plans
  • Cultural humility, empathy, active listening, patient-centered advocacy, and problem-solving skills
  • Adaptability, initiative, and collaboration within multidisciplinary teams
  • Accordingly, commitment to ethical and community-centered service delivery
  • Valid driver’s license and access to reliable transportation
  • Willingness to participate in home visits, community engagement, and occasional evening/weekend work
  • Current Vulnerable Sector Check and up-to-date immunizations, including TB screening
