Job Summary:

Lac Seul First Nation is seeking a dedicated Youth Transition Worker to join the Post Majority Department. Under the supervision of the Director of Post-Majority Services, the Youth Transition Worker provides culturally appropriate, needs-based support to youth and young adults transitioning out of care. Moreover, the focus of the Youth Transition Worker is to help youth build independence by accessing essential services, developing personalized transition plans, and strengthening connections to family, community, culture, and support systems.

Location: Lac Seul, ON
Term: Permanent Full Time
Salary: Starts at $27.94/hour *based on qualifications and experience.
Hours: 35 hours per week

Job Duties (include but are not limited to):
  • Firstly, conduct needs assessments and create individualized transition plans in collaboration with youth.
  • Assist youth in building practical life skills such as budgeting, employment readiness, and goal setting.
  • Also, provide culturally appropriate, wraparound supports to youth and young adults transitioning from. care (ages 18–26).
  • Support youth in accessing housing, education, financial assistance, health, wellness, and cultural services.
  • Help youth understand their rights and navigate child and family services processes.
  • Provide advocacy, service navigation, and connections to local, regional, and federal programs.
  • In addition, build trusting relationships and strengthen connections to family, community, culture, Elders, and Knowledge Keepers.
  • Maintain strict confidentiality, cultural safety, and accurate documentation.
  • Collaborate with First Nations services, agencies, and partners to strengthen supports and reduce service gaps.
  • Coordinate and organize program-related events and activities.
  • Perform other related duties as assigned.
Qualifications:
  • Grade 12 (OSSD) or equivalent.
  • Post-secondary education in social work, health, business administration, or office administration is an asset.
  • 1–2 years of experience in case coordination, service navigation, administrative support, or related work in Indigenous or community-based settings.
  • Knowledge of community history, family relations, and cultural context is an asset.
  • Experience working with First Nations communities and people.
  • Experience working with youth or young adults, especially those involved in Child Welfare.
  • Ability to create individualized plans and support youth with goal setting and life skills development.
  • Knowledge of community resources related to housing, education, health, and cultural supports is an asset.
