About the Job:

Join the Canadian Mental Health Association Team to become a Family Care Coordinator today! The Family Care Coordinator is responsible for working within an interdisciplinary team setting within the Coordinated Specialty Care model NAVIGATE. The FCC will also provide assessment, psychoeducation, support and advocacy to support persons and family members of clients affected by early psychosis. The Family Care Coordinator will provide crisis support and intervention as needed and will utilize both individual and group programming to deliver these services.

  • Part-Time Permanent – 1 Vacancy, 42 Hours Biweekly
  • Rate: $$32.61 – $34.63/ hour (based on experience)
  • Start Date: As Soon As Possible Application Deadline: May 29, 2026
  • This position is within OPSEU/SEFPO Local 720 Bargaining Unit
Your Job Duties?
  • Firstly, initial intakes and assessment interviews with family members
  • Subsequently, orient families to services, engagement and maintenance of therapeutic relationships
  • Identifying goals and interventions with family members
  • Delivering ongoing support based on needs and goals, with emphasis on communication, problem-solving, decision making and recovery-oriented care.
  • Similarly, provide psychoeducation to family members re: psychosis, etiology, treatment options, recovery processes and care giver wellness, using the Navigate Framework.
  • Information sharing, liaison and advocating between the client, family, other members of the treatment team, and the larger treatment community
  • Also develop and delivering group programming for families
  • Public/community education and training as requested
Additional Duties:
  • Additionally, supervise students as requested and appropriate
  • Participating and contributing to program development, evaluation and research as requested and appropriate to role
  • Likewise, attend supervision, team, administrative and relevant system-level meetings
  • Adhering to and promoting CMHA’s values and Code of Ethics Policy
  • Complying with CMHA’s Health and Safety Policies
  • Finally, complete other duties as assigned
What do you bring to the job?
  • Nursing Diploma/Degree or Social Work Degree with an emphasis on providing care within a preferred psychiatric or medical setting. Other combinations that reflect an integration of medical and therapeutic knowledge will be considered
  • Significant training and experience working with families and young people in the health/mental health field preferred
  • Knowledge of family systems theory, client and family-centered practice and the adaptation process in families when faced with illness
  • Health teaching experience with adults helpful
  • Ability to work in a collaborative partnership with clients, family, team and community
  • Excellent skills in the areas of communication, engagement, crisis intervention , critical thinking and solution focused problem solving.
  • Knowledge of community resources to support clients and families, i.e. counselling services; emergency services; income support, etc.
  • Computer experience with Microsoft Outlook/365
  • Deescalation Potentially Violent Situations
  • Thorough understanding of privacy legislation and mental health act
  • Commitment to on-going professional development and personal growth
  • Eagerness, enthusiasm, positive attitude and commitment to the principles of hope and recovery
  • We prefer proficiency in spoken and written French.
  • Legally eligible to work in Canada
  • Current Police Vulnerable Sector Check
  • Valid Class “G” Driver’s License and reliable vehicle with $2,000,000 liability insurance
